How to Access our Services
Services can be accessed through self-referral or third-party referral using our REFERRAL FORM. The manager will be able to answer any of your questions and arrange a suitable time to meet with you.
The NDIS self-directed funding model allows Participants control over major decisions, including their required service provider or services.
Steadfast Care Services Partners with you and supports you in managing your funding package. The appropriate manager will respond to all referral enquiries on the same day they are received.
A representative from Steadfast Care Services will arrange a time and place to meet with you to discuss your needs and goals and how our services could support you.
We will create a personalised plan with you and agree on the nature of the support we will provide. We will maintain regular communication with you and consistently review and update your plan.
